If you have had a lighter laser treatment and thought it helped but did not really fix the problem, that reaction is worth taking seriously. Treatments like MOXI® and BBL® HEROic™ do what they are designed to do, but they have a ceiling. A TRL laser exists for the concerns below it: deep acne scarring, etched lines around the mouth, and decades of accumulated sun damage.
This is not an argument that everyone should consider deeper resurfacing. It is for the smaller group who genuinely need it.
What Separates a TRL Laser from the Lighter Options
TRL stands for Tunable Resurfacing Laser, and both of our deepest treatments run on that erbium platform. The difference from HALO®, MOXI®, and BBL® is how deep the laser reaches.
The lighter treatments work at or near the surface, which is why they ask for little recovery. Contour TRL™ and ProFractional® are ablative and reach into the dermis, where scar tissue and static lines are structured. That depth is what makes real correction possible, and it is also what attaches real downtime.
Contour TRL and ProFractional Solve Different Problems
These two are often mentioned together, but they suit different patients.
ProFractional® treats the skin in a fractional pattern, creating narrow columns of deep treatment while leaving the tissue between them untouched. Those bridges speed healing, which lets ProFractional® reach a depth appropriate for real scar correction without a vacation-length recovery. It is our primary tool for acne scarring, surgical and C-section scars, and deep wrinkles.
Contour TRL™ resurfaces the entire treated area rather than a fraction of it, with your provider dialing in the exact depth in microns. It is the strongest non-surgical correction we offer and one of the few treatments that meaningfully softens the deep vertical lip lines injectables cannot fully address. It is also frequently performed around the mouth and eyes during facelift surgery, treating texture while the surgery addresses the structure underneath.
If You Suspect You Need Less, Not More
These are the deepest two of four resurfacing options we offer. NanoLaserPeel® and MicroLaserPeel® use the same erbium technology at lighter settings, handling dullness, rough texture, enlarged pores, and uneven tone with far less recovery. Our companion post on the lighter peels covers both, and our laser skin resurfacing overview lays out all four depths.
Who Is Genuinely a Good Candidate
Most patients who choose deep resurfacing have tried something lighter and wanted more correction. The strongest candidates have deep static wrinkles, significant sun damage, or established scarring, and can plan around the recovery rather than squeeze it into a busy stretch.
Several things deserve a candid conversation first. Active breakouts, cold sores, or infection in the treatment area need to be resolved beforehand, a history of keloid scarring calls for extra caution, and recent isotretinoin use should be disclosed. Because these are ablative treatments, deeper skin tones carry a higher risk of pigment changes at deeper settings than our non-ablative options do. That does not rule anyone out, but settings and aftercare deserve a longer discussion.
What Recovery Actually Requires
ProFractional® asks for 7 to 10 days. Makeup goes back on after 24 hours, most patients return to normal social activity within a few days, and daily sunscreen is essential for at least four weeks.
Contour TRL™ asks for two weeks and often longer, and how much longer depends heavily on the depth chosen. Expect redness, swelling, and peeling through the first several days, with visible redness lasting eight to twelve days. Most patients are comfortable wearing makeup at about a week. How long residual pinkness lingers depends almost entirely on depth, and your provider will tell you what to plan for before you book. Direct sun needs to be avoided for two months, with daily sunscreen for up to three. Collagen remodeling continues for three to six months, and that is when the final result emerges.
Why Fall Is the Practical Window
Sun avoidance is not a suggestion here, which is what makes timing matter. Autumn on Long Island brings shorter days and less sun, making the aftercare far easier to follow than it would be in July, and it leaves runway to heal before the holidays. If you are considering deeper resurfacing this year, this is the last realistic window.
Let Us Tell You Which One You Need
Choosing between these two, or deciding whether a staged approach makes more sense, is not something to sort out alone. At North Shore Cosmetic Surgery in Melville, NY, the team, led by board-certified plastic surgeon Dr. James Romanelli, will examine your skin, listen to your goals, and give you a straight answer about what will actually correct the concern you have been living with.
