Eyelid Surgery: What Blepharoplasty Actually Does and Who It’s For

Have you noticed people telling you that you look tired, even on days when you feel perfectly rested? Or that applying makeup to your upper lids has gotten harder because there is less visible lid to work with? Those are two of the most common reasons patients come to our Melville, NY office asking about eyelid surgery. Heaviness above the eyes tends to develop slowly, which is part of why it catches people off guard. Nothing dramatic happens on any given day. Then at some point the mirror simply looks different.

What Eyelid Surgery Actually Addresses

Blepharoplasty, the clinical term for an eyelid lift, is really two related procedures. Upper eyelid surgery removes excess skin and sometimes a small amount of muscle or fat from the upper lid, which reopens the eye and restores a more defined crease. Lower eyelid surgery addresses puffiness, under-eye bags, and crepey skin below the eye, often by repositioning fat or adding fat by fat grafting rather than simply removing it. Some patients benefit by one; others benefit by both. Which approach makes sense depends entirely on your individual anatomy, not on a standard formula.

The Functional Side of Upper Eyelid Surgery

This is the part most people do not expect. When upper lid skin descends far enough, it can encroach on your upper field of vision. Patients often describe it as needing to tilt their head back slightly to read, or noticing that the top of the road disappears when they drive. Many also compensate without realizing it by holding their brows up all day, which contributes to forehead fatigue and end-of-day heaviness around the eyes.

When the obstruction is measurable, an ophthalmologist can document it with a visual field test.  Insurance may provide coverage, but typically only if the upper eyelid skin hangs over the upper eyelid lashes!

Do I Need Eyelid Surgery? Signs Worth Noticing

There is no single age at which this becomes relevant. Some patients are in their thirties, noticing upper lid hooding for the first time. Others are in their fifties or sixties with skin that has begun to affect how well they see. Signs that tend to bring people in include:

  • Upper lid skin resting on or touching the lashes
  • Lifting the brows unconsciously to keep the eyes open
  • Eyeshadow or liner disappearing into the fold
  • Looking fatigued in photographs regardless of how you feel
  • Hooding that worsens noticeably by evening

Eyelid Surgery or Something Else?

Not all upper-eye heaviness comes from the eyelid itself. Sometimes the brow has descended, and lowering it further by removing lid skin would work against you. A simple way to think about it: if gently lifting the outer brow with a fingertip resolves what bothers you, a brow lift may be the better answer, either instead of or alongside blepharoplasty. Anesthesia for Eyelid Surgery

Upper eyelid lifts can be performed using a local anesthetic for patients who prefer this approach.  Otherwise, a light general anesthetic is used, as an outpatient procedure.  This is always recommended for lower eyelid surgery.

What Eyelid Surgery Recovery Realistically Looks Like

Expect visible bruising and swelling for roughly ten to fourteen days, with the most noticeable phase in the first week. Fine sutures typically come out within about a week. Most patients are comfortable returning to desk work within seven to ten days, though we ask you to hold off on strenuous exercise and heavy lifting a bit longer. Cold compresses, elevation, and patience do most of the work.

Incisions sit in natural creases, so they tend to settle discreetly, but they need consistent sun protection while they mature. Final, natural-looking results continue to refine over a few months. This is part of why late summer and fall are popular for facial procedures on Long Island. Staying out of strong sun is easier in cooler weather, and patients planning ahead for the holidays have room to heal comfortably.
